There's two different mechanisms of dialysis. One is called "hemodialysis," the other one is "peritoneal dialysis." Essential the purpose of dialysis is to remove waste products from the blood and also to remove excessive fluid from the blood when required. We now have a new therapy available known as "ultra-filtration" where we can use a machine to remove fluid in patients who are resistant to conventional therapy or resistant to diuretics. Dialysis and ultra-filtration are two different modalities. Dialysis removes waste products and also removes fluid. Ultra-filtration in some ways with heart failure only removes extra fluid. Typically when someone requires ultra-filtration for congestive heart failure, the treatment typically last for 24 to 72 hours and it allows us to remove 5 to 30 pounds of fluid in that time frame. So the data that most of the patients can be maintained quite well without further treatments for more than 90 days. Again, it really depends on how bad the person's heart is, how compliant they are with their diet in terms of following a low sodium diet, and how compliant they are with their other medications, their other heart medications. When patients develop kidney failure, there's two different mechanisms for kidney failure, one is worsening of a chronic kidney disease, so they have known kidney problems for years and the kidney function declines. Typically when that happens the kidney do not recover because whatever is causing their kidney to fail, whether it be diabetes or high blood pressure or combination of both, is continually pounding on the kidneys and the kidneys cannot recover. That typically requires hemodialysis which is performed 3 times a week in an out-patient setting for 4 hours per treatment, or they can do a home dialysis treatment called peritoneal dialysis, which is done daily typically at bedtime while they are sleeping. Somebody who develops acute kidney injury or acute renal failure, where the kidneys suddenly shut down, often sometimes just need dialysis as a bridge until their native kidneys recover.
