A Nurse Navigator is a nurse that follows patients once they go from the inpatient to the outpatient status. We follow the patient, we call them at home, make sure that they understand the discharge instructions, we go over those with them if we need it. We make sure that if there's a follow up appointment that needed to be done that the doctors recommended why the patient was here. We made those appointments for them or we make sure that the patients are going to be attending to those appointments. The benefits of having a nurse navigator is it's statistically proven that I think 30% of all the patients actually understand their discharge instructions—only 30%. So when we call these patients, a lot of them have questions regarding what was happening during the hospitalization, what the doctor's instructions were, what the nurses' instructions were, it could be regarding wound care, it could be regarding the medications...and by us calling them, we can answer most of those questions and we can also facilitate the follow up with their doctors. Sometimes they don't remember exactly what doctor they saw, they don't remember the name or the face or they remember the face but don't remember the name...so we can help them, you know, facilitate the follow up. The Nurse Navigator also interacts with the patient's family because a lot of these patients need assistance to get to their appointments. So a lot of the times, we end up talking to the family members that take care of the patient at home, there may be even a care giver. We even encounter with them about the medications of the patient, how they need to be taken, and when those appointments are, we give them this information to this patient, the person that is actually taking the patient to those appointments so that it's not missed.
If we didn't have a nurse navigator at the facility, a lot of these patients would be sort of lost when they leave the hospital. Let's say they started a new medication that was very important and the patient really didn't realize that they were supposed to be on that medication, or they needed a very important blood test that needs to be rechecked in 3 days... they did not remember any of that. So it's very important when you have somebody that checks that after you leave the hospital, makes sure you understand what you're supposed to do as your follow up care, if it ends up on the reduction of readmission of the patients back to the hospital. I would recommend Westside Regional because number one, the doctors that we have here I think they are very caring, very knowledgeable. My specialty's is cardiac and I have worked with these cardiologists for a long long time, so I would recommend any of them. Secondary, the program like this is not offered in every hospital and by having it, it really benefits the patients when they go home...it benefits the patient, benefits the doctor that know that the follow up they recommended is going to be taken care of, benefits the hospital because also the reduction of the readmission it's something that we look forward.
